DENTAL RECORD SYSTEM OCAMPO-ESQUIVEL DENTAL CLINIC

1. Introduction

Dental Record system can be defined as a series of software that provides all medical and administrative procedures to be done in the computer environment in Mouth and Dental Health. An assessment of the state of an individuals dental development can be important in the evaluation of his or her overall maturity and can be used to compare growth and health between individuals and populations.

In addition, we provide this kind of system to develop the life cycle of new growing dental clinic to give them good benefits to their company. It will develop solutions that meet the needs and desire of the owner. In designing of this system we think how can be used to improve safety of their records. The dental record, a complete record keeping system of through and accurate dental forms, dental charts, filing products, and more. Accurate patient record keeping in relation to routine dental care is necessary to ensure appropriate and systematic flow of the treatment plan.

Dental records should provide relative status to the patients concern and requests, the proposal treatment plan, and the treatment performed, and should include all supporting documentation. Any departures from expected outcomes should be advised of comprised results as soon as the dentist is sure of the facts, in these circumstances, all information presented to the patient must be documented.

Dental records can be used to identify the medical and dental history of the patient, the clinical conditions, records on-ongoing oral/dental health care, record financial information and identify, when appropriate, nature and level of patient expectations. The patients have right to expect that their dental health information will be kept confidential and review and obtain a copy of their dental records including consultation reports of other practitioners.

It is appropriate to share dental and medical records with other health professionals for the purpose of assisting in providing optical care. The dental records should only be disposed of in manner that ensures that the confidentiality of the information is maintained.

1.2 Statement of Objectives The objective of this project is to help the clinic to save time on accessing their records,

space on their clinic and to introduce information technology to the clinic.

1.2.1 General Objective This project will create a system that can help to reduce the paperworks, can create a fast access on the records of the clinic and can save and delete a particular data.

The specific objectives are as follows: -To reduce data redundancy -To easily find and access data -To avoid the paper works as much as possible to save space on the clinic -And to introduce information technology in the company -To create a system that can easily manipulate by the user

1.3 Significance of the Study

The significance of this study discuss the importance of the proposed research and its relevance. This investigation is relevant for theory, practice and future research. The investigator explain why it is important for the study to be undertaken and indicated the likelihood of its contribution to the advancement of knowledge.

Due to the increasing number of patient of Ocampo Esquivel Dental Clinic, this study helped the school regarding their activities and transactions. The proposed design aimed to benefit the clinic in their facilities such as maintaining the files especially dental records.

The benefit of the study is Ocampo Esquivel Dental Clinic would be adapting the innovation of technology through application program of database that will make their work with excellent performance. The develop system will be handling the flows , files ,records and the problems that the clinic is encountering. This will help the clinic in maintaining the data of the patients and the clinic will not use the manual data gathering. The clinic will just check the system for the records needed.

Course Content

Dental Hygienists are employed in public, private and specialist dental practice where they collaborate with Dentists and other dental care workers. They are responsible for providing preventive and therapeutic dental hygiene care. This involves the provision of evidenced based dental hygiene treatment including the taking of medical histories, patient examination, hygiene treatment sequencing, patient education, fissure sealants, restoration polishing, professional tooth whitening, splinting of mobile teeth, removal of sutures, exposing and interpreting intra-oral radiographs, administering local anaesthesia, placing temporary restorations and using therapeutic agents including fluoride to prevent or control oral disease and the non-surgical management of periodontal conditions including root surface debridement. The Dental Hygienist also provides services to residents of aged care and supported residential facilities and contributes to the oral health awareness of other health care providers.

1.4 Scope and Limitation of the study

Ocampo Esquivel Dental Clinic has manual operations on patient management records. We have proposed a system that will ease the employees on operating. This system is a computerized dental record that tends to be a part of a local stand-alone dental information system that allows storage, retrieval and modification of records.

This requires computer-literate operators to avoid system malfunctions. We have assumed that the users have no knowledge about using database systems. We will conduct training to familiarize users on how to operate database systems. The dental clinic has a low count of patients so there will be minimum records in the database. Searching and updating records will not be hard to do. The operation that will be done is fast and accurate due to less record count.

Using this system is far better that paper-based record. Paper based records are still the most common method of recording patient information for most hospitals and practices in the U.S. The majority of doctors still find their ease of data entry and low cost hard to part with. However, as easy as they are for the doctor to record medical data at the point of care, they require a significant amount of storage space compared to digital records.

Using this system to read and write a patient's record is not only possible through a

workstation but depending on the type of system and health care settings may also be possible through mobile devices and other gadgets that are connected to the main computer.

3. Data Gathering Procedures and Output We have established collaboration with Ocampo - Esquivel Dental Clinic from Antipolo City. After conducting an interview with the dentists and assistants, we have collected the relevant and substantive data. Upon collecting the qualitative data derived from the clinic, careful analysis has been done to prepare and analyze how to best customize the course to the target patient populations.

These data are used to create a system to replace manual operation of patient records and management. We have already collaborated on a project to develop a system for dentists and assistants for a better patient record management.

During the proposed visit, we will investigate the application if it needs improvement or its better to keep it simple and easy to use. However, using the system typically requires knowledge about databases and how to use them. When the system is installed in the clinic, it needs a regular maintenance to avoid errors.

4. The Existing System

4.1 Company Background

The establishment of Ocampo- Esquivel Dental Clinic was conceiving during 1998.After graduation in Centro EscolarUniversity, they planned to start their own clinic with the help of their parents. In their first clinic, they have an associate dentist with Dra. Daing. They had a good working relationship until Dra. Daing decided to go abroad in 2000. As the new owner of the clinic they decided to transfer the clinic at the bigger space which is located at Antipolo City. During that time they invited Dr. Corminal to be their associate dentist, up to now he still working in the clinic. They started with one dental chair and a secretary. The clinic is doing well and going steady. Every single day they had walk-in patient, experiencing different tooth related problems and different kind of patient attitude towards dental care. They said when your new on this kind of service, waiting and boredom is one of the problems you will encounter. But as time goes by and as your patient grew in numbers you will be the one having problem how to accommodate all your patients, but they manage to attend every one of them without compromising the dental service work. With the growth in numbers of the patient, Dr. Esquivel decided to have one clinic with two dental chairs so that they can attend to their entire patient.

In 2003, they renovated the clinic and transfer Dr. Esquivel's dental chair and all his equipment in their clinic. They hired a second dental assistant to help on the growing responsibility in the clinic. After a year Mr. and Mrs. Esquivel plan to have their own house and their own cars. This year they put up a second clinic with two dental chairs and all necessary dental

equipment. And they invited Dr. Polillo and his wife to be their associate dentist. For now the new clinic is going steady and doing well and the dental service continues.
