Please bring your instrument with you unless you are playing piano or drums. Please complete this form, have your Pastor sign it, and bring it with you on May 12th. This will allow us to plan appropriately for you to attend as well as allow us to contact you with any additional details. Commitment If selected to be a part of the Section 4 Youth Worship Team, I commit to: Be faithful in attendance to practices and services for which I am scheduled. Promptly notify the proper person if I am unable to attend a practice or service for which I am scheduled. Follow the Illinois District platform standard (listed below) when ministering with the Worship Team. Practice the selected music on my own time. Commitment to the Worship Team is for one year. Each person on the Worship Team will minister at approximately half of the services and should be at all practices. (We can schedule the services you minister in around work commitments if we know about it well in advance.) Typically, there will be one practice per month. District Platform Standards Modest dress for all, including: No cap sleeves or sleeveless blouses. Good necklines on all dresses, etc. Hemlines that are at least to base of knee. Mens dress shirts that are appropriate dress type.

Modest personal appearance for all, including: Men that are clean shaven, having no facial hair such as mustaches, goatees, beards, pork-chop sideburns, etc. Men that have short, neat haircuts (off of collars and ears). No excessive jewelry. No make-up. Women that have long, uncut hair.
