RASHTRIYA SWASTHYA BIMA YOJNA Introduction


The Rashtriya Swasthya Bima Yojana (RSBY) is a national health insurance scheme launched in 2008 by government of India to provide health insurance to households living below poverty line (BPL) in order to protect them from major health shocks that involve hospitalization. RSBY was launched in June 2009 and Durg was the first district to be enrolled under this scheme.

Health Insurance works by spreading the risk of healthcare expenses over a number of people, So that the financial burden of unfortunate few who need healthcare is shared by the many fortunate who remain healthy.

Most of us, at some point in our lives, will need to seek expensive healthcare services e.g. hospitalisation or surgery.

A few days of hospitalisation, major surgery or chemotherapy can cost lakhs of rupees. Health Insurance protects us against unexpected financial loss. It empowers us to access and afford needed healthcare services.

RSBY is meant for the poorest and aims to relieve them of the burden of healthcare costs. However, our findings suggest that patients still incurred out-of-pocket expenses. Furthermore, most private hospitals did not admit patients under RSBY beyond their BPL quota. A lack of transparency at healthcare facility level is evident as a large number of persons were not aware of the amount deducted from their RSBY cards. There is need to enhance transparency and proactive disclosure by healthcare facilities for patients and for effective analysis of the scheme. There is need to strengthen government healthcare services and regulate private healthcare services in order to have desired results of RSBY.

EXAMPLE
Mr. A has fallen ill and requires hospitalisation, which will cost Rs. 10,000. There are two different ways to cover this cost: Collect, or pool, a small amount of money from each person in the community to cover the cost of As hospitalisation, or Make A cover the cost on his own which could potentially push him into poverty and set the precedent that everyone must pay their own healthcare costs, regardless of how high they may be. Each person in the community makes a small contribution and eventually the community shares the burden of the large expense incurred for hospitalisation of one.

Rashtriya Swasthya Bima Yojna (RSBY) is a health insurance scheme. launched on 1st April 2008 by the Ministry of Labour and Employment (MoLE). The goal of RSBY is to provide health insurance coverage to individuals and households below-the-poverty-line (BPL). RSBY plans to cover the entire BPL population in the country (approx 30 crore) by 201213.

Enrolment of poor in the RSBY in its third year of operation in the states does not shoe any sign of it reaching all the poor by 2012. Only kerala shows some commitment largely because since early 2006. It is difficult to assess the performance of the state in terms of hospitalisation rates as the states average under RSBY being closed. It is also the time for state Govt to take a closer look at the way premiums are set.
