Safety I says improvement comes from learning from accidents and incidents.
Safety II says improvement comes from learning from when things go well.
The world has changed beyond recognition over the past fifty years, but our approach to workplace safety hasn't kept pace. It is impossible to have a culture of continuous improvement with a management system based around failure avoidance.
Safety v2.5 asks us to start over with fresh eyes and explore new ideas. How can safety management become fit for the modern workplace?
