Authorization Letter For Mobile Equipment Collection

This authorization letter allows a named proxy to collect mobile equipment purchased by the customer. It provides the customer and proxy's names, ID numbers, and dates of issue. The proxy must present this letter, the customer's original ID, and the proxy's original ID for verification when collecting the purchase. If any documents are missing, SingTel Mobile reserves the right to withhold the equipment, and may cancel the transaction if not collected on time.

I confirm that the authorized person named above will have on hand the documents set out below for verification by your staff : -

1. This Authorization Letter duly signed by me 2. My Original NRIC / Passport / Foreigner ID 3. My Appointed Proxys own original NRIC / Passport / Foreigner ID

In the event that any of the above documents is not produced, I agree that SingTel Mobile reserves the right not to handover the purchase (i.e handphone) to the proxy. In addition, SingTel Mobile reserves the right to cancel the purchase/transaction if the item(s) are not collected within the stipulated timeframe by the registered customer/proxy.
