The College of Nurses of Ontario (the College) collects the information in the QA Program Learning Plan Form if you have been selected for Practice Assessment
under the general authority of the Regulated Health Professions Act, 1991, S.O. 1991, c. 18, the Nursing Act, 1991, S.O. 1991, c. 32, and its regulations; and the
Colleges by-laws. The College collects the information for the purpose of assessing your continuing competence through its Quality Assurance Program. Appropriate
measures are taken to safeguard the confidentiality of the personal information you provide and all documents become the property of the College.

What learning needs did I identify through practice reflection and peer input?
Identify a minimum of two (2) to three (3) learning needs.
Throughout my student practice, which includes my pre graduation
simulation I feel that some learning needs include, MSK ROM, symptom
analysis, and palpation.

Learning Goal #1
My goal must be based on my learning needs.
My goal within the pediatric clinic is to improve my musculoskeletal
assessment; specifically ROM by taking on more MSK related injuries that
come into the clinic and to properly assess the range of motion of my
patients by the end of week 11.

Goal #1: Activities and Timeframes
How am I going to achieve my goal? Identify a minimum of three (3) learning activites with timeframes/timelines.
I am going to achieve my goal by doing the following (4) learning activities:
(1): Review range of motion techniques within previous notes/documents in week 8
(2): Seek advice from other nurses and preceptor on ways they use to remember steps to assess MSK injuries as
well as their techniques in week 9
(3): In week 10 I plan to research more on range of motion specific to different MSK breaks
(4): In week 11 I will prefect my range of motion assessment by incorporating all learning activities

My goal must be based on my learning needs.
My second goal is to properly palpate all the appropriate areas during a
MSK crisis by week 11.

Goal #2: Activities and Timeframes
How am I going to achieve my goal? Identify a minimum of three (3) learning activites with timeframes/timelines.
I plan to achieve my goal by:
(1): Reviewing my notes on palpation regarding MSK in week 8
(2): Continuing to assess MSK patients and sit in on more experienced nurses to gain knowledge based on their
assessment
(3): By week 11 be able to palpate in a circular motion, being sure to compare bilaterally to all the appropriate
areas

My goal must be based on my learning needs.
My third goal is to properly do a symptom analysis on patients to set aside
the true cause between fractures, bone shatters, and bone bruises.

How am I going to achieve my goal? Identify a minimum of three (3) learning activites with timeframes/timelines.
I am going to achieve my goal by:
(1): Studying from the Assessment text on the symptoms for MSK injuries in week 8
(2): Apply my knowledge to my pediatric practice with the help of my preceptor in week 9
(3): Differentiate how each situation should be handled so that I can properly act on MSK injuries sucessfully in
week 11

My learning goals support my commitment to continuing competency
because they allow me to take initiative to further my knowledge base
in struggling areas, which in-turn will better my nursing practice.

What did I learn?
What impact has my Learning Plan had on my practice?
My learning plan has impacted my practice largely due to the fact that my
goals allow me to be more sensitive to detail. In MSK injuries the smallest
differences contribute to different outcomes as well as different treatment.
It has also taught me that not all injuries should be treated the same and the
importance of bilateral palpation.
