This is to certify that ANNIE CAMBA CAGUIAT, a regular-permanent Teacher I (TVE


DEPARTMENT) at B. S. Aquino National High School, Concepcion, Tarlac, was not
required to secure a permit to study while taking up her masteral studies at Luna
Colleges, Inc. in Tayug, Pangasinan.

In this connection, she need not to secure a permit to study because her studies do not
interfere with her work.

This certification is issued upon verbal request of Mrs. Caguiat for all legal intent and
purposes it may serve.
