Create a database named My Business_your name on your flash drive which includes the
following: (1)

A Table that contains the following fields

o ID, Surname, Given Name, Address,

Phone Contact, Account Balance.

o Set the Primary Key to the ID field. (1)

o Set the Field size for Surname and Given Name at 15 (1)

o Set the Field size for Address at 35 (1)

o Create an Input mask for the Phone Contact Field. (2)

o Save the Table as Customer Information. (1)

Enter information for ten (10) customers in the table. (2)

A Form named Customers that includes all of the above information. (1)

A Query called Customer Query that includes: ID, Surname, Given Name, Address,

Phone Contact. (3)

A report called Customer Report (2)

Format the Form and Report to suit your taste. (2)

Total 20 marks
