Over the past years PMI has substantially simplified the online PMP Exam Application form.

We have therefore decided that the "Experience Verification Worksheet" is no longer needed
to assist our students in preparing their PMP Application

We recommend that you download the official PDF version of the application form and use
this as a GENERAL guideline:
