GLASCOW COMA SCORE(GCS)

EYES
Open – Eyes open spontaneously
To Voice – Eyes open when you call their name
To Pain – Eyes open to painful stimuli
No Response – Eyes do not open to pain or voice

VERBAL
Orientated & Alert – Knows current date, time and place
Confused – Not orientated to time or place
Inappropriate words – Words to not correspond to questions asked
Incomprehensible – Moaning, groaning sounds
No Response – No verbal words or sounds

MOTOR
Follows commands – Able to perform task asked ie: squeeze my hands
Localises to Pain – Moves hand/arms towards pain site ie: sternal rub
Withdraws to Pain – Moves away from pain site ie: moves finger away from nail bed
Pressure
Decorticate flexion – Flexes both arms together
Decerebrate Extension – Extends both arms together
No Response - Does not move to command or painful stimuli

PUPILS
+ = Reactive to light
- = Not reactive to light

GCS SCORE (3-15)


Severe brain injury < 8 (Needs endotracheal intubation)
Moderate brain injury 9-12
Minor brain injury >13
