A. For All Applicants:
   1. The printed Application Form from your online application
   2. 2 pcs. 2"x2" ID picture
   3. Parents'/Guardians'Proof of Income (whichever is applicable)
> Latest Income Tax Return (ITR) of parents or guardian
> Ce i cate of Tax Exemption from the Bureau of Internal Revenue (BIR)
> Ce i cate of Indigency with Gross Annual Income of parent/guardian issued by the Barangay Chairman
> A davit of No Income or Case Study from DSWD
> For children of Overseas Filipino Workers (OFW) and Seafarers, a latest copy of contract or proof of income
may be considered

B. For Incoming Freshmen:

   1. For Graduating Senior High School applicants – (a) 1st Semester Senior HS Grades AND (b) Ce i cate of
Good Moral Character
   2. For Senior High School Graduates – (a) Form 137 AND (b) Ce i cate of Good Moral Character

1. Fill-in the Scholarship Application Form online at h ps://www.chedro8stufap.com (Application period is from February
1, 2019 to April 30, 2019 only)
2. Print your accomplished application form from your online application.
3. Check the status of your online application (whether it's uploaded in the system or not). Posting is done daily (except

4. If your name is on the list of successfully uploaded applications, prepare the above-mentioned documentary
requirements to be submi ed to CHED Regional O ce VIII (a er 3 to 5 days)
