Radio Show Risk Assessment Form

The document is a risk assessment form for recording a radio show at the Oakes Radio Studio. It identifies four potential risks: equipment getting broken if wires are dangling; people tripping on wires and getting hurt; electrical fires from bringing drinks into the studio; and breaking the microphone from loud noises. For each risk, it proposes control methods like securing wires, warning of tripping hazards, only drinking outside the studio, and warning interviewees about loud noises near the microphone. It also provides a risk rating matrix to evaluate the probability and severity of each risk.
