SUDS Worksheet

This document provides instructions for a student to select one of three anxiety-producing scenarios and help the individual create a 5-10 item anxiety hierarchy including subjective units of distress (SUDs) ratings from 0-100% for each item. The scenarios involve a mother's anxiety about sending her child to school, a client's driving anxiety after a minor car accident, or a teenage client's anxiety about starting school with new dental braces. The student is to use the provided matrix to list distressing thoughts or events and their corresponding SUDs ratings to help conceptualize the individual's anxiety.
