RE : Request for Payment Extension


Dear John:

As of ___________________, records show we still have to settle the


amount of ___________________ as remaining balance for the above-
mentioned loan due on ___________________.

However, because of the business reverses that we experienced, I


foresee the inability to pay the balance on the due date. It is on this note that I
request your assistance to possibly arrange for extension of payment on the
remaining balance for the said loan.

It will be greatly appreciated if I can be allowed to pay the balance in six


(6) equal monthly installments of ___________ beginning ___________, and
every month thereafter until the account shall have been fully paid.
