Promissory Note

August 1, 2019

I, Ginalyn G. Lagarde, of legal age, a student of the University of Eastern Philippines, College of
Veterinary Medicine, DVM – 6, residing at Purok 2 Barangay Cangcahipos, Laoang, Northern
Samar, do hereby execute this note and promise to pay back, in full, the VTH Clinics Fee for the
upcoming semester amounting to P3, 500 to be paid in full before the said semester ends.

I would like to ask for your kind consideration to allow my enrollment for the first semester of
SY 2019-2020 despite the non-payment of the said fee. I am only having financial difficulties at
the moment due to lack of financial support from my family but promise to pay it once I’m able
and I assure you that such will be settled before the semester ends.
