Using This Spreadsheet
Risks Are things that could go wrong, but aren't happening at the moment. They're things you need to keep an eye on
Issues Are live problems: things you need to sort out, requests etc. They tend to have a lot of actions or steps involved to solve them.
Actions Are single step todo items: practical steps to sort issues. Tracked elsewhere: it's more detail than managers or execs need to know about.
1 Identify risks at the start of the project, and enter them in the risks sheet
2 Rate the risks against the scales for impact and likelihood. You should not be using 5s very often AT ALL. That's a crisis number. Refer to
the scales below for what the ratings mean
3 Review the Risks log regularly: perhaps fortnightly. Are there new risks? Are any of the risks now happening? Have the likelihoods or
impacts changed?
4 Enter and rate new risks. Number new risks sequentially - the top of the first column should show you the current highest
5 If Risks are happening, mark as issue and start creating lists of actions to tackle it.
6 Action items go in to todo lists or a seperate actions list, assigned by person and due date
Status
Risk Could happen in the future. Requires a trigger to become an issue
Issue An active problem - a risk that has come about. Start an action list for it!
Resolved A solved risk: one that isn't a problem any more
Closed All done with!
Impact
Additional Cost Schedule Overrun Safety Implications For a project it means…
Water down your Extra work to fix, but won't impact time /
1 Very Low < 1% 1 day
shirt costs
Schedule overrun, cost overrun, but we can
2 Low 1-5% < 1 week Needs a plaster
absorb it.
Will involve annoyed client, asking them for
3 Medium 6-10% 2 weeks Trip to minor injuries more time and money, possibly not breaking
even. Possible Legal issues.

4 High 11- 20% 1 month Call an ambulance
money and reputation. Legal action
We're all fired, the company goes under,
5 Very High > 20% > 1 month Death

Likelihood
Probability Feels a bit Like…
1 Very Low 1 in 100 Unlikely. Pobably won't happen Seven heads in a row when tossing a coin
2 Low 1 in 10 Possible, but not likely Drawing an Ace from a pack of cards
3 Medium 1 in 5 Feasible, but not certain. Rolling a six on a dice
4 High 1 in 2 Probable. 50/50, toss a coin. Tossing a coin and getting heads
5 Very High ≥ 1 in 2 Inevitable. Near Certainty Yup.
