We agree to give you the benefits sot out in your Polcy Data Page as explained inthis Policy Booklet forthe regular premiums paid by you. ‘The Application for PruLink Insurance, the Pokcy Data Page, the Sales Ilustration Form and this Policy Baoklet constitute the entire insurance contract. Any Form and Application Form for Top-Up, Fund Switch, Withdrawal, Premium Redirection, Inerease/Decvease In Regular Premium or Inccease/Decrease in Sum Assured which may be Issued during the ife ofthe Polcy also becomes part ofthe entre insurance contract, “The information you gave us in your Application Form, any Form and any correspondence for this Policy were raed on by us in deciding whsther or nat to insure you. Your Policy Data Page indicates the Ptan that you have. Your policy will be void If your age at the Policy Etfectivity Date is less than eighteen (18) years, Your Policy is only for the benefit and the period shown on your Policy Data Page. Ifwe receive your Regular Premium after 12:00 noon | Monday, we use the price ‘on Thursday of the following week. PY How is the fund valued? 3.4 PRU LIFE UK. How DOoEs YouR POLICY WORK? \wmen you apply for your palcy, you decide on the amount of regular premium that you want to pay. The annualized regular promium must be equal o or more than the minimum amount Wwe specty at the time of application. We reserve the right to increace or decrease tho ‘minimum amount of Regular Premium payable. ‘You also decide on the Mode of your Regu Premium payment. We reserve the right not to ‘accept that portion of the Regular Premium which exceeds the maximum acceptance limit of \We show your Modal Regular Premium on your Policy Data Page You can choose to invest in any of the funds that we offer for investment. A description lof each fund offered can be found in Section 4 of this Poley Boke. When you apply for your Policy, assuming that the funds are available, you must ‘choose whether you went: (a) allyour Regular Premiums to be invested in one (1) ofthe funds; or (&) your Regular Premiums tobe invested in more than one (1) fund ‘Your Regular Premiums are used to buy units in the fund or funds you have chosen, We assign the units to your Policy. Policy Fee. This is used to cover administrative expenses. The amount of policy fee I stated in your Sales tlustraion Form. Insurance Charge. This is used to cover the cost of providing the sum assured in the event the Life Insured dies or a portion of the sum assured in the event the Life Insured becomes totaly and permanently disabled. The rate varies according to the age of the Life insured at each biling date. Your inltial insurance charge is stated in your ‘Sales llustation Form. This charge also includes the cost of Insurance taxes due on this poly. Rider Charge. This is used to cover the Optional Benefits chosen by you. Similar to insurance charge, the rate varies according to the age of the Life Insured at each biting date, Extra Charge. Any extra charge is used to cover the increased mortality or ‘morbidity associated withthe risk the Company is exposed to, The rae varios by age ofthe Life Insured and the substandard risk class where the Life Insured is classified \When the unit price is calculatod, we will deduct from the fund an Annual Management ‘Charge (AMC). The current AMC at the time of your application is stated in your Sales Mustration form. Wuat Type Or Funps? “The folowing separate funds ere available fr this policy: ‘2) PRULink Bond Fund — seeks to achieve an optimal level of income in the medium term together wih lang term capital growth through investments in fixed income secures and money market instruments b) PRULInk Managed Fund — seeks to optimize medium to long term capital and income growth through investment in fixed income securities, money market instruments and shares listed on the Philippine Stock Exchange. ©) PRULink Growth Fund ~ seeks to optimize medium to long term capital and income growth, wth an emphasis on strong capital growth, through a greater focus of investment in shares of stocks listed in the Philippines. The fund also invests in fixed income securities, and money market instruments, ‘We may introduce new funds in the future to give you a wider choice of investment. 'We have sole discretion in determining how the funds are invested and the forms of investment. Our investment policy has been approved by the Insurance Commission. No change in our investment policy will be done without the prior approval of the Insurance Comission. Your Policy Data Page shows you the fund where your Policy is linked, Your obligation 54 Failure to pay - units 52 jin your account Failure to pay- no 53 units in your account Payment OF Recutar Premiums Your Policy Data Page shows the date your fst Regular Premium is due, the amount ‘of Regular Premium and the frequency (Mode) of Regular Promium paymonts. You must pay us the first Regular Premium before the Policy Effectiviy Date. Thereafter, you must pay us ‘the Regular Premiums within tht one (31) days ofthe date they are due. We send you a nouce when your Regular Premium is due. However, you must ensure that your regular premiums are paid on time even f you do not receive the notice \We send you a reminder notice if your Regular Premium isnot received by a particular date However, you must ensure that your regular premiums are paid within thity one (31) days after they are due even if you do nat recsive the reminder notice, It you fall to pay a Regular Premium when due or you discontinue paying Regular Premiums ‘and there are sufcient units in your account, your polcy automatically continues at the same ‘sum assured for the same benefits, \We continue to bil you forthe charges and the costs of providing you the same benefits We send you a reminder notice before your units become insufficient to pay forthe charges. When there are insuficient unis in your account, your policy terminates immediately onthe day the charges are due. We refund to you the value ofthe Insufficient units. If you fall to pay a Regular Premium on time and there are no units in your account, your Policy terminates immediately when the unpaid premium is not received within ‘ity - one (81) days fom the day the Regular Premium is due. ene gO erey Charges Right to vary charges 6 61 62 How Do We Bit You? From the first Regular Premium due date of your policy and every month thereafter, ‘we bill your account for a policy fee, insurance charge and other costs of providing you ‘the Core and Optional Benefits. We create a palicy debt corresponding tothe monthly charges. while there arene unis in your account provided you pay the Reguiar Premiums on tme, and there are no withdrawals made from your account, under Sections 7.11end 7.12, ‘The policy debt 's accumulated in your account without interest ‘At each month whan there ara units in your account, we use the uni price atthe next pricing dato to soll proportionate units fram all the funds to pay for the policy debt, policy tee, ineurance and rider charges and other applicable charges. ‘We wil terminate your policy i ther i insufficient unit value to pay for tha policy dab, policy fee, insurance and rider charges and other applicable charges. However, wo will not terminate your policy on account of an outstanding policy debt during te frst four (4) years from the Policy Effectvity Date, provided you pay the Regular Premiums on time and there are no withdrawals made ftom your account, under Sections 7.11 and 7.12, ‘We reserve the right to vary the policy fee, insurance and rider charges but not more then the ‘amounts shown below. The percentage of the new regular premium invested under the Policy shall be based on the Regular Pramium Allocation Schedule shown in the Policy Data Page, with the years counted from the date the first new regular premium is due. | you increase your regular premium, you may or may not choose to increase the sum assured. However, if your current sum assured is less than five (5) times your new ‘annualized regular premium, we wil automatically increase your sum assured to that amount, ’An accompanying increase in the sum assured as a result ofthe change in regular premium wil be reflected on your new Policy Data Page. To increase your regular premium, you must use our appropriate application form and meet the conditions stipulated on it. We wal notify you if we accept your application. Your Transaction Confirmation Advice shows how much of your new regular premium in ‘ particular year is used to buy units, You can reduce the amount you pay as regular premium but the reduced amount must bbe equal fo or more then the minimum amount we specify. The reduction starts from the date your next regular premium is due. After that, you pay the reduced regular premium every {ime your regular premium falls due. Reduction of regular premium is allowed only beginning on the second policy year you reduce your regular premium, you may or may not choose to reduce the sum assured for any of the benefit amount(s). An accompanying reduction in the sum assured or benefit amount(s) 2s a result ofthe change in regular premium wil be efiected on your new Policy Data Page. To reduce your regular premium, you must use our appropriate application form and mest the conditions stipulated on it. We wil naiy you fwe accept your application, You can make an adaltional one-off payment (called top-up), subject to Company’ rules and regulations, if the Life Insured is within tho acceptable ago limits. Wo provide the Lif Insured with a doath benefit of one hundred twanty five percant (125%) of your top-up. If you make a withdrawal onthe units from your top-up, we deduct one hundred twenty five percent (125%) ‘of the withdrawal amount from the death benefit corresponding to the top-up, “The amount deducted wil nat exceed one hundred twenty five percent (125%) of te top-up. When you apply for your top-up, provided that the funds are available, you must choose whether you want: 2 all your top-up o be invested in one (1) ofthe funds; or your top-up tobe invested in more than one (1) fund. “The minimum top-up amount is PRP20,000. We reserve the right not to accept that portion of, the top-up which exceeds the maximum acceptance limit of our reinsuer. “The price used to buy units depends on the timing when we receive your top-up. We will deduct the ‘costs of providing you the sum assured by seling proportionate unis from all your fund ‘equivalent to the insurance charge. it you are required to pay extra for medical, occupational ‘or hazardous activites, this will be added to the costs. Every month thereafter, wo use the unt pice atthe next pricing date to sell proportionate tunis rom all your funds to pay forthe costs of providing the sum assured. We dotermine the costs after aking into account the ineurance rates then in use and the age ofthe Life Insured at each biling, ‘To make a top-up, you must use our appropriate application form and meet the conditions ‘stipulates on I. We will notify you if we accept your application. ‘You are not allowed to make a top-up if you fall to pay any ofthe regular premiums due during the fits tree (3) years of the policy. Any top-up made during this period willbe used fist {pay any regular premiums in arrears and the reguler promium due. Any remaining amount, ‘within the minimum top-up requirement, will be used as top-up.
