Greetings!
We are 2nd year BS Fisheries students from the College of Fisheries Sto. Tomas La
Union. We are currently conducting our descriptive research which aims to gather data
about the job preferrences of male students in our respective institution, a requirement for
our STATS 101 subject.

I understand that the researchers will collect my personal information to cater to


my inquiry, and that such information is stored in the database once collected. The use,
storage, access and disposal of personal information are governed by Data Privacy Policies.
Providing information means that I am giving my full consent to the researchers to collect
my information and expressly waive my rights under the Data Privacy Act of 2012 (R.A.
