Affidavit of Loss for MRBS Policy

This affidavit of loss was submitted by an individual who is a member of the Philippine Public School Teachers. They had been issued a Certificate of MRBS Policy which has since been lost. The individual's house underwent renovations in 2004 where important documents were stored in a filing cabinet, which was then moved to another location. When attempting to retrieve the certificate later, the individual was unable to find it, so they are submitting this affidavit to attest that the certificate has been lost in order to use the document for its intended purposes.

AFFIDAVIT OF LOSS

I, (NAME)Filipino, of legal age, Married, and a resident of (ADDRESS) under oath


declare that;

1. That I am a member of the Philippine Public School Teachers as such I was issued a
Certificate of MRBS Policy

2. That sometime last 2004, our house was renovated and that the filing cabinet where
my important personal document are kept including the said policy was likewise
trasnferred to some other place;

3. That when I tried to retrieve the same, my search prove futile, hence, considered lost;

4. That I am executing this affidavit to attest the veracity of the foregoing statement and
for whatever purpose this may serve.
