Carmen, Davao Del Norte. Cause of death: Septicemia & Severe Community Acquired
Pneumonia. She started as a borrower of Asset Credit & Loans, Inc. since October 20, 2011
for the amount of 3,000.00 with the term of sixty days. This client has a good record in her
account.

Kindly see attached file as a requirement to settle the account of the said client.
