favor is hereto attached as Annex “A”.

This is to request your good office to revert Tax Declaration No.


___________, in the name of its previous owner, the late ___________.
A copy of the old Tax Declaration in the latter’s name is hereto attached as
Annex “B”.

It must be noted that the property covered by the above-mentioned Tax


Declaration was the subject of Civil Case CA-GR CV No. ___________,
decided by the Honorable Court of Appeals in favor of the late
___________. Copy of the said Decision dated ___________ is hereto
attached as Annex “C” to “C-8”.

It must likewise be noted that the said Decision already became FINAL
AND EXECUTORY on 01 August 2002. To prove the same, a copy of the
Certificate of Finality issued by RTC ___________, Branch _____, is
hereto attached as Annex “D”.

In view of the foregoing, may I humbly reiterate the heirs’ request to


revert Tax Declaration No. ___________, in the name of its previous
owner, the late ___________.
