ANNOUNCEMENT
REGISTRATION FOR THE 18-MONTH MEDICAL EXAMINATION AND TREATMENT
PRACTICE PROGRAM FOR MEDICAL DOCTORS AT CHO RAY HOSPITAL
Pursuant to the Government's Decree No. 109/2016/ND-CP dated July 1, 2016 stipulating the
issuance of practice certificates for practitioners and operation licenses for medical examination
and treatment establishments.
Pursuant to Circular No. 21/2020/TT-BYT dated November 30, 2020 of the Minister of Health
stipulating the practical guidance for the issuance of general medical examination and treatment
practice certificates for medical doctors.
Cho Ray Hospital hereby announces the "Registration of the 18-month medical examination and
treatment practice program for medical doctors at Cho Ray hospital" as follows:
1. Course Information:
- Category: Medical doctors
- Practice period: 18 months
- Admission circulation: continuously
- Admission period: at the beginning of each month (Note: 18-month polyclinic class admission is
only open when the number of students is sufficient).
- The registration of medical examination and treatment practice’s scope falls into one of three
categories: General Medicine, Internal Medicine, and Surgery.
- Practice guidance facility: Cho Ray Hospital, Children's Hospital 1, Hung Vuong Hospital.
- Upon completion of the course, the hospital will issue the following certificates: Certificate of
practice in accordance with Form 02 as specified in Appendix I of the Government's Decree
109/2016/ND-CP
2. Admission document composition:
- Application form for practice at medical examination and treatment establishments (according to
form 109/2016/ND-CP)
- Copy of medical doctor's degree (notarized copy) with the original copy for comparison
- Photocopied ID card (notarized copy)
- Resume (verified by local authorities)
- 3x4 photo (04 copies) with full name written in the back of the image
- Tuition: 3,000,000 VND/student/month
- Students can pay tuition fees by cash, card or bank transfer and 1 full payment must be made in
advance.
