Sleep Study Result

The patient underwent a sleep study which showed severe obstructive sleep apnea with an AHI of 48.9 events per hour. Various respiratory parameters including oxygen saturation were monitored during sleep. Positive airway pressure titration successfully treated the apnea and other respiratory events at a pressure of 5 cm water. The study results confirm a diagnosis of severe obstructive sleep apnea.

SPLIT-NIGHT POLYSOMNOGRAM REPORT

Procedure: Central and occipital EEG, EOG, submentalis EMG, thermocouple, nasal
pressure, ECG, thoracoabdominal motion, anterior tibialis EMG, snore sensor and
pulse oximetry were monitored. The tracings were interpreted based on the American
Academy of Sleep Medicine (AASM) V2.6 scoring manual. Positive airway pressure
titration was started after 2 or more hours of diagnostic polysomnography when
significant hypopneas and apneas were detected. Epworth Sleepiness Scale prior to
this study was 12/24. BMI of 21.6 kg/m2.

EEG and Sleep Analysis: Typical sleep characteristics such as sleep spindles and K
complexes were noted. There was no voltage asymmetry, epileptiform patterns nor
electrographic seizure discharges noted. Sleep efficiency was 84.4 % for the
diagnostic study and 95.5% for the therapeutic portion. Sleep latency was 10.2
minutes during the diagnostic study. For the breakdown of sleep architecture during
the diagnostic study, 36.1% was spent in stage N1 sleep, 54.1% in stage N2 sleep and
0.0% in stage N3 sleep. REM sleep had a latency of 88 minutes and comprised 9.9%
of TST during the diagnostic study. Arousal Index in the diagnostic study was 44.8/hr.,
which decreased to 9.2/hr. in the therapeutic study.

Respiratory Analysis: The AHI (Apnea Hypopnea Index) for the diagnostic study was
48.9/hr. Majority of the respiratory events were obstructive apneas. Mixed apneas and
central apneas were not observed in the study. From a baseline oxygen saturation of
97% while awake, during sleep the mean oxygen saturation was 95% with a nadir of
88%. Moderate to loud snores were noted during the study. Positive airway pressure
titration was tested from 4 cm water to 7 cm water. At a pressure of 5 cm water, using
a medium size nasal mask (Airfit N20 by ResMed), the respiratory events, snoring and
desaturations were corrected.

EKG Analysis: Average heart rate during sleep was 54.6 bpm. There was a decrease
in heart rate variability noted during the titration study. PVC’s and PAC’s were not
present in the study.

Leg Movement Analysis: Periodic limb movement index was 6.2/hr in the diagnostic
study and 1.9/hr in the therapeutic study.

Interpretation: The sleep study is diagnostic of severe obstructive sleep apnea.


Snoring, respiratory events and oxygen desaturations were corrected using a
continuous positive airway pressure of 5 cm water. Please correlate clinically.

Obstructive Sleep Apnea (OSA) in Adults:
Normal: AHI <5 per hour
Mild: AHI of 5 to =15 per hour
Moderate: AHI of 15 to =30 per hour
Severe: >30 per hour
