EACH PLAYER/PLAYER'S GUARDIAN

I, ___________________________Filipino and resident of Barangay Macaas,


guardian of _____________________with address at __________________ in
consideration of and as a condition of the acceptance of my
participation in the 2nd Mayor Avis G. Monleon and SK Inter-Barangay
Tournament 2023-2024, hereby waive release and discharge
Barangay Macaas Council and SK Council, Baskethall League staff,
volunteers in the facility utilized by the baskethall league, from all the
actions suits, claims and or demand,criminal.civil or whatever nature
arising out of, or in any way connected from participating in the said
event.

I am aware that participating in this event may involve risks of serious


injury, or even death from various cases including, but not limited to;
falls, over exertion, dehydration, physical contact with other participants,
spectators and as well as effects of weather.

I voluntarily assume all risks associated in joining this basketball league.

I hereby declare that I have read and fully understood this instrument
before signing the same willingly and voluntarily, and with full knowledge
of the legal effects of the same.
