Daily Monitoring

This daily monitoring log allows a person to track their experiences throughout the day including who they interacted with, what they did and said, how others responded, their physical sensations, thoughts, and subjective units of distress or discomfort level (SUDS) on a scale from 1 to 100. The purpose is to gain self-awareness and identify patterns that could help address social anxiety issues.
