CASE ANALYSIS
Susanna Kaysen is 18 years old at the time of admission to the facility:
- She had a recent suicide attempt by downing a bottle of aspirin with a bottle of
vodka.
- She loses track of time and has an unstable train of thought, this subsequently
Presenting causes troubles in forming sentences and finishing conversations.
Problems - The patient has trouble being present because of lapses in her memory and
consciousness.
- The patient engages in inappropriate and promiscuous behavior with various
people, regardless of their relationship to her circle.

Developmental History: Susanna is a relatively normal high school student with


inclinations towards writing.

Educational Background: She is a recent high school graduate with prospects of going
to a good university due to her parents’ social standing.

Occupational Bacground: N/A

History of Present Illness: Susanna’s symptoms manifested clearly at the age of 18.
Examples that were clearly shown were her disinterest in social conventions,
promiscuity, and suicidal tendencies. She also manifests dissociative symptoms and

reckless behavior which does not register to her as being “negative”.

History of Psychiatric, Psychological or substance use problems: The patient has no


recorded substance use problems; however, it has been noted that she attempted
suicide using a copious amount of aspirin and vodka.

of care: Susanna was rushed to the ER to flush her system and was subsequently
referred to a psychologist, who referred her for admission to McLean Hospital.

Susanna had experienced a major life event (graduating from high school) while
Predisposing Factor having bouts of depression, possibly also brought about by the feeling of
purposelessness and lack of direction after graduation.
The breakdown before her suicide attempt occurred after one of her sexual partners
pleaded to engage in sexual intercourse with her quite aggressively after she had
Precipitating Factor already declined. The client had also begun to experience bouts of depression again
after Lisa had disappeared from the institution, leaving her stressed and worried
about her friend’s state and whereabouts.
Her apparent dependence on Lisa and resistance to treatment have been hindering her
Perpetuating Factor progress. The client acts as if she’s above therapy and as if it wouldn’t help her to
divulge anything to the therapists due to Lisa’s influence.
The client has a relatively good support system in the form of her parents, who
Protective Factor supported her admission to the facility. She is also a well-educated woman with an
inclination towards writing.

PROPOSED TREATMENT
Susanna Kaysen is a relatively functioning being who would highly benefit from
schema-focused therapy, which is focused on identifying unmet needs that led to
negative behavior. This could also work in congruence with free association and
psychodynamic therapy due to mentions of childhood neglect that had been buried in
the subconscious of the patient. The main treatment can also be dialectical behavior
therapy, since this would help the client manage stressful life events without resorting

to extremes.

Although medication has been prescribed to the patient, it is highly suggested that her
dose be gradually reduced until she becomes independent of them again since the
patient shows great signs of recovery through regulation of her emotions, which will be
highly achievable through therapy.

Through Cognitive processing therapy, the client will gain an in-depth understanding
Final Goals of posttraumatic stress disorder (PTSD), its underlying symptoms, and how it affects
one’s functioning. After the 12-session, the client will gain skills on how to change and
challenge maladaptive thinking patterns and unhelpful beliefs related to the trauma.
The client will develop a new understanding and conceptualization of the traumatic
event; thus, it will reduce the adverse effects of trauma on his current situation.
