Graduate  Program  Admission  
By   signing   this   form,   I   agree   and   authorize   the   UP   Asian   Institute   of  
Tourism  (UP  AIT)  to  collect,  store,  process  and  if  needed,  share  with  UP  Diliman  
offices   with   a   need-­‐to-­‐know   basis,   my   personal   information   as   compiled   and  
detailed  in  my  application  to  the  Graduate  Program  of  UP  AIT.  
In   the   event   that   my   application   to   the   Graduate   Diploma   in   Tourism  
Development   and   Management   (GDipTDM)   has   been   accepted,   I   am   allowing   the  
UP   Asian   Institute   of   Tourism   (UP   AIT)   to   post   my   full   name   in   the  
announcement  of  results  of  admission  to  the  program  (both  for  print  and  online  
channels).    
This  is  also  to  authorize  UP  AIT  to  use  the  information  I  provided  in  this  
application   form,   including   information   provided   in   the   attachments   to   process  
my   admission   to   the   University   of   the   Philippines.     This   consent   form   is   being  
given  in  compliance  with  the  requirements  of  the  Data  Privacy  Act  of  2012.
