I / We, the applicant / co-applicant have applied for the Product Name loan with YES BANK
and understand that as per the RBI guidelines “Master Direction - Know Your Customer (KYC)
Direction, 2016”, every applicant is required to submit Officially Valid document ( OVD ) in
support of the current address proof for purpose of opening account.

List of OVDs are as under:-

 Passport  Driving Licence
 Voter's Identity Card issued by the  Job card issued by NREGA duly signed
Election Commission of India by an officer of the State Government

 Letter issued by National  Aadhaar number + E-KYC


Population Register containing authentication or Offline verification**
name and address **Offline verification of Aadhaar number
would include Aadhaar Letter/Card,
Downloaded Aadhaar (e-Aadhaar), Aadhaar
secure QR code and Aadhaar paperless
offline e-KYC

My/our current address is not updated in any of the acceptable OVDs as mentioned above. Hence
I/we have submitted copy of _________________________________ to meet the address proof
requirement, and I/we undertake to submit self-attested copy of one of the OVD as listed above
with updated current address proof within 3 month of disbursement.
