CF-JPIA Week 2024

Consent Agreement:
By signing this Data Privacy Consent Form, I acknowledge that I have read, understood, and
agree to the processing of my personal data by CF-JPIA in accordance with the provisions
outlined below.

Purpose of Data Processing:

I consent to CF-JPIA collecting, storing, and processing my personal data for the following
purposes:
 Registration and participation in CF-JPIA Week 2024 events.
 Communication regarding event updates, schedules, and related information.
 Documenting and archiving participation records for event management and reporting
purposes.
 Sharing relevant event-related information with accredited/affiliated third parties or
independent/non-affiliated third parties as necessary for the successful execution of CF-
JPIA Week 2024.

Types of Personal Data Collected:

The personal data collected and processed by CF-JPIA may include, but is not limited to, the
following:
 Full name
 Date of birth
 Contact information (address, phone number, email address)
 School affiliation (local chapter)
 Photographs or video recordings taken during event participation

Data Protection Measures:

CF-JPIA implements appropriate technical and organizational measures to safeguard the
confidentiality, integrity, and security of personal data collected and processed. These measures
include, but are not limited to, encryption, access controls, and regular data backups.

Data Retention and Disposal:

I understand that my personal data will be retained for the duration necessary to fulfill the
purposes outlined above or as required by law. CF-JPIA will securely dispose of my personal
data in accordance with applicable data protection regulations once it is no longer needed for the
stated purposes.
Revocation of Consent:
I understand that I have the right to revoke my consent for the processing of my personal data at
any time by submitting a written request to CF-JPIA. However, revocation of consent may
impact my eligibility to participate in CF-JPIA Week 2024 events.
