CHANGE OF NAME DEED (Deed Poll)
THIS CHANGE OF NAME DEED is made this 10th September 2019.
By me the undersigned YOUR NEW NAME of 36 Example Drive, Exampleville, AB1 2CD now or lately
known as YOUR OLD NAME.
IT IS WITNESSED AND HEREBY DECLARED as follows:
1. I absolutely fully and entirely renounce relinquish and abandon the use of my said former name of
YOUR OLD NAME and assume adopt and determine to take and use from the date hereof the name
of YOUR NEW NAME in substitution entirely for my former name of YOUR OLD NAME.
2. I undertake that I shall at all times hereafter in all records paperwork deeds documents and other
writings and in all actions and proceedings as well as in all dealings (formal or otherwise) and
transactions and on all occasions whatsoever use and subscribe the said name of YOUR NEW NAME
as my name in substitution for my former name of YOUR OLD NAME so relinquished as aforesaid
to the intent that I may hereafter be called known or distinguished not by the former name of YOUR
OLD NAME but by YOUR NEW NAME only.
3. I authorise and require all persons at all times to designate describe and address me by the adopted
name of YOUR NEW NAME.

and also my said former name of YOUR OLD NAME and have affixed my seal the day and year first above
