Mircera Algorithm
Background:
• Mircera is generally administered every two weeks or less frequently, and dosing is tied to the patient’s hemoglobin result. Mircera will be
administered to all patients on the same dialysis day of the week, which is based on the facility weekly lab draw schedule. (See Lab Draw/Dose
Administration Schedule.)
• Exception to this is the New Patient:
o If a first dose of Mircera is to be administered less than or equal to 3 days before the facility Lab Draw/Dose Administration Schedule,
wait to administer the dose per the Lab Draw/Dose Administration Schedule.
o If the first dose of Mircera is to be administered more than 3 days before the Lab Draw/Dose Administration Schedule, one dose of
Mircera may be given on another day of the week per algorithm, but subsequent doses of Mircera should be administered according to the
facility Lab Draw/Dose Administration Schedule.
▪ The patient’s subsequent dosing schedule will be based upon the patient’s order frequency and the Lab Draw/Dose
Administration Schedule.
o Note: At least 11 days must elapse since the patient’s last dose of Mircera.
At least 7 days must elapse since the patient’s last dose of Aranesp.

• Spectra Hgb value used in calculating new Mircera dose must not be older than 7 days.

When a Mircera dose change is indicated per algorithm, dose calculations and Mircera order will be entered into the electronic system and documented
appropriately in the patient’s medical record. The new Mircera dose will be administered when the next scheduled dose is due following the receipt of the
Spectra Hgb result per the Lab Draw/Dose Administration Schedule.

Patient New to Dialysis: • If TSAT is less than 20% and Ferritin is greater than 1200 ng/mL, consult the Physician/NP/PA.
Iron Evaluation and Administration • Otherwise, if Hgb is less than 10.0 g/dL and either TSAT is less than 20% or Ferritin is less than 100 ng/mL,
▪ Do not start Mircera until the patient has received a 1 gram iron repletion sequence per signed iron
algorithm or Physician/NP/PA order.
▪ If during administration of the 1 gram iron sequence, Hgb values decrease by greater than 1.0 g/dL, consult
with Physician/NP/PA regarding earlier initiation of Mircera.
▪ Upon completion of the 1 gram iron sequence, recheck Spectra Hgb at next scheduled facility lab draw.
If Hgb is less than 10.0 g/dL, start Mircera according to Starting Dose Chart.
Note: If patient has been on Aranesp, administer Mircera no sooner than 7 days after last Aranesp
dose per the facility Lab Draw/Dose Administration schedule.
Patient New to Dialysis – Starting When either TSAT is 20% or greater and Ferritin is 100 ng/mL or greater,

Patient has received a 1 gram iron sequence:
1. If Hgb is less than 10.0 g/dL: Administer Mircera per Starting Dose Chart.
2. If Hgb 10.0 g/dL or greater: Do not begin administration of Mircera. Start Mircera when Hgb
is less than 10.0 g/dL per Starting Dose Chart.

General Orders:
Weight to Determine Starting Use patient’s post weight to determine starting dose. If post weight is unavailable, use patient’s estimated dry weight.
Dose Note: Round patient’s weight to nearest kg, up or down (i.e., 74.4 kg is rounded to 74 kg; 74.5 kg is rounded to 75 kg).
Prior to Calculating Mircera Review last available Spectra Hgb result prior to calculating Mircera dose.
Dose
Route of Administration All Mircera doses will be administered IVP.
Dosing Frequency Mircera is administered every two weeks unless otherwise noted in the Maintenance Dose Chart.
Frequency of Dose Change Mircera dose changes will be made every 28 days using Maintenance Dose Chart unless patient meets exception criteria
(see Exceptions to Monthly Dose Changes).
• If the correct prefilled syringes per the Prefilled Syringe Selection Chart are not available for the ordered Mircera
Missed Dose Due to Mircera
dose, delay the administration until the next scheduled dialysis treatment when the correct prefilled Mircera
Prefilled Syringe Availability syringes are available.
• If the correct Mircera syringes are not available and dose is delayed for more than 1 week, contact
Physician/NP/PA for order.
• Continue to follow the facility Lab Draw/Dose Administration schedule.
Minimum Dose Per Treatment The minimum dose of Mircera is 30 mcg. If the calculated dose is less than 30 mcg/every 2 weeks, follow the
Maintenance Dose Chart to change frequency.
Minimum Days Between ESA • At least 11 days must elapse since the patient’s last dose of Mircera.
Doses • At least 7 days must elapse since the patient’s last dose of Aranesp.
Mircera® on Hold and No Open If Mircera® is on hold and there is no open Mircera® order in the last 3 months, when the Hgb is less than or equal to
Mircera® Order in Last 3 Months 11.1 g/dL contact the Physician/NP/PA to obtain a re-start dose.

Identification of Lowest Effective Dose: • Discuss with the Physician initiation of the CMAB Recommended Hyporesponse Evaluation Algorithm.
Patients at maximum Mircera dose of • Patients on the maximum Mircera dose for 3 consecutive months with: (1) a TSAT of equal to or greater than
225 mcg every two weeks for 3 30% and (2) a Ferritin greater than 200 ng/mL and (3) a Hgb greater than or equal to 10.0 g/dL and (4) not
consecutive months requiring a blood transfusion:
a. Continue to follow Physician ordered iron algorithm.
b. Decrease Mircera using Column 3 of Maintenance Dose Chart.
c. Every month thereafter, reduce the dose using Column 3 of Maintenance Dose Chart until the Hgb falls
below 10.0 g/dL, and then increase the dose to the previous prescribed Mircera dose.
• Patients on the maximum Mircera dose for 3 consecutive months with: (1) a TSAT of equal to or greater than
30% and (2) a Ferritin greater than 200 ng/mL and (3) a Hgb less than 10.0 g/dL and (4) not requiring a blood
transfusion:
a. Continue to follow Physician ordered iron algorithm.
b. Decrease Mircera using Column 3 of Maintenance Dose Chart.
c. Every month thereafter, reduce the dose using Column 3 of Maintenance Dose Chart until the Hgb drops by
0.5 g/dL or greater, and then increase the dose to the previous prescribed Mircera dose.
Changing From Every 2 Week to Every When dosing is changed from every two weeks to every 4 weeks, administer the next dose 3 weeks following the last
