The OMR Sheet contains provision for answering 100 Nos. of Multiple Choice Questions from (1) English (2) Physics (3)
Chemistry and (4) Biology/Mathematics. Candidates are directed to attempt English, Physics and Chemistry compulsorily
and any one between Biology and Mathematics. They are to attempt either Biology or Mathematics.
Important Instructions to the Candidate
1. You should bring only this Admit CARD and BLACK BALL POINT PEN to the Examination Hall, along with any Photo ID proof.
2. Communication devices like cellular phone, pager, Bluetooth Headphones and any kind of smart gadgets etc. are not allowed in the examination hall.
3. You have to keep all the material outside the examination centre at your own risk.
4. Darken only one circle in the OMR Sheet completely for each answer.
5. No Candidate will be allowed to leave the hall before completion of the examination.
6. Ensure that you have correctly filled up your Roll No., Question Booklet Number and Version Code. Answer Sheet Number and signature at the space provided in the OMR
ANSWER SHEET and QUESTION BOOKLET.
7. This Admit Card is to be retained by you until the admission process is over.
8. All candidates shall be present at the Examination Venue thirty minutes before the scheduled examination time along with the Admit Card.
9. The doors of the Examination Hall shall be opened 30 minutes before the commencement of Examination in each shift. No candidate shall be allowed to enter Examination Hall
15 minutes after the commencement of the Examination in each shift.
10. A candidate must sit in his/her allotted seat only.
11. During examination, candidates are required to put his/her full signature on his/her Admit Card and Attendance Sheet as well as affix thumb impression on the Admit Card and
Attendance Sheet at designated places.
12. Candidates must bring their own black dot pen. Possession of any book, note, scribbling or other objectionable materials in the Examination Hall will make the candidate liable
for disqualification.
13. No replacement of OMR Answer sheet.
14. No cell phone is allowed inside the Examination Hall/premises of the Examination Centre.
15. No calculator/electronic gadgets are allowed inside the Examination Hall and premises of the Examination Centre. Presence of any such item shall make the candidate liable
for disqualification.
16. A candidate shall not help or try to help any other candidate or obtain or try to obtain any help from other candidate in choosing answers. In case of any need, a candidate
should simply stand up to draw the attention of the invigilator. If any candidate is found to be talking or copying from fellow candidate, he/she will be warned by putting a `W` in red
colour and later if found to repeat the same, will be liable for expulsion.
17. Adoption of unfair means in the Examination Hall by any candidate shall automatically cancel his/her candidature of the entire examination.
18. Do not use eraser (solid or liquid) to correct any Answer as this will damage the OMR Answer Sheet resulting in the computer rejecting the Answer or the whole Answer Sheet.
19. Candidates are warned that any attempt to use unfair means in the examination or any breach or attempted breach of examination rules shall make them liable to expulsion.
20. Notwithstanding the issue of the Admit Card the Officer-in-Charge shall have the right for reasons which may appear to him sufficient to cancel the examination of any
candidate before, during or after examination.
21. After completion of the examination candidates must submit BOTH Question Booklet and OMR Answer Sheet to the invigilator.
22. The decision of the Srimanta Sankaradeva University of Heath Sciences, Guwahati with regard to conduct of the examination shall be final and binding to the candidate.
23. Candidates will not be allowed to go to toilet within first hour of commencement of examination and during last 1 hour of the examination.
