Have decided to opt for Transfer of Service to PAKISTAN AIRPORT Authority

consequent upon Functional Segregation of Pakistan Civil Aviation Authority pursuant to

implementation of Pakistan Civil Aviation Authority Act 2023 and Pakistan Airports

Authority Act 2023 and accept the terms & conditions specified therein in addition to

applicable rules and regulations at my own free will. I hereby confirm and agree that my

option for Transfer of Service is irrevocable, if accepted by the Competent Authority.

Please read the below statements carefully and mention the date with signature.

1. I understand that my decision regarding Transfer of Service as indicated on the

application is voluntary and at my own free will and shall not retract my decision
after submitting this signed application form, however, Competent Authority shall
have the right to accept or reject the same.

2. I hereby confirm that I have read and understood all terms and conditions of
Transfer of Service including applicable rules and regulations and will accept the
decision of the Competent Authority, whatsoever.
