1. Apoorav (name of Candidate) aged 25 years son of Sh. Umesh Kumar (father's name) presently resident
at V.P.O. Spail & Tehsil Nagrota Surian, Distt. Kangra HP do hereby solemnly declare and affirm on oath as
under:
(i) That I bavc gonc through all the cligibilityconditions of the Prospectus for Counselling for
PG (MD/MS/DNB) degree courses and appiying for admission to postgraduate MD/MS
Degree Courses (Allopathy) for the SESSION 2024-2025.
(i) That I am not in the service of Government of Himachal Pradesh on regular/ adhoc/
contract/ RKS basis.

That I am working on Regular/ Adhoc/ Contract/RKS basis as In-Service Medical Officer but
not possessing the requisite qualifying service as defined at clause 2 and its sub clauses. I
understand that my candidature be considered as a Direct Candidate in terms of clause 3 as
already claimed by me in application form. I shall produce a relieving letter/NOCfrom the
government at the time of admission.
