Evolution of Mega Prosthesis in Orthopedics

Mega prosthesis is a device used for reconstructing large bone defects caused by trauma or tumors, moving away from traditional amputations to limb salvage techniques. The evolution of these implants has led to the use of advanced materials like stainless steel and titanium, though affordability remains a challenge. Future advancements include 3D printing of patient-specific implants and increased public awareness to improve accessibility and treatment outcomes.

What is mega prosthesis

Its a device which is used to reconstruct any large defects in the

bone due to various conditions like trauma periprosthetic fractures
and largely used in diseased bones like tumors and bone cancers
Earlier days the standard treatment for bone cancers use to be
amputations which is a social stigma and often affects mobility and
psychological behavior
Hence the limb salvage has gained popularity lately
Today there is lot of development in terms of designing to
functionality to suit various needs
Evolution of designs and materials
Various types of implants

Ever since the need for end-prosthesis has increased there is

constant quest for development of a reliable reproducible and
patient specific implants with high durability and cost effectiveness
currently stainless steel titanium and cobalt chromium and
polyurethelene are materials used to produce these implants
How ever these are still very expensive and affordability is still a
challenge to many
Various applications

Most of these implants are used to reconstruct large bony defects

caused by trauma or loss of soft tissue stabilizers
They also are used in revision surgeries in failed total knee and hip
surgeries
Musculoskeletal disorders
Bone transplants
Application in musculoskeletal oncology

Many aggressive tumors like osteo sarcoma ,Ewings sarcoma,Giant

cell tumors
Once diagnosed these tumors are treated with Neo adjuvant
therapy with various chemo agents
Followed by limb salvage surgeries are planned
Followed by chemotherapy or rt and follow up of the patient is very
Important
Protocols and planning

Key to these surgeries is pre op planning

Pet ct ,ct scans mri and X-rays are standard diagnostic tools
Depending on tumor staging and nature of tumor timing of
surgeries shall be planned
Age and demands of the patients is very important
Affordability and availability and metastatic disease will have major
deciding factors
Should be done in well equipped facility with surgeons who are well
versed with the protocols and proper training
Challenges

The biggest challenge currently is awareness

Availability of implants
Affordability
And longivity of the implants
Infections
Recurrence
Multiple surgeries
Current concepts

Patient specific implants

3d printing
Anatomically designed implants with better kinematics and better
materials
Multidisciplinary approach with combined therapies
Affordable implants
Future concepts

Instituional 3d printing of the models and implants

Public awareness in partnership with governments
More surgens to be trained and shall set treatment protocols
Better materials with better designs
More cost effective treatments
Few mega prosthesis designs
The bold adventurer always succeeds the best
