SmartSets: Well Child Check

The TogetherCare team has built a new workflow that uses an Epic SmartTool called SmartSet. The purpose
is to streamline the provider workflow to compliant documentation with appropriate charges. SmartSets are
preconfigured visit templates that help you quickly select diagnoses, write orders, choose a level of service,
and more. Please refer to the Physician Personalization Guide on your learning home dashboard for more
information on how to customize your SmartSets.

The following are the new SmartSets referenced by this tip sheet:
• Pediatric WCC Newborn
• Pediatric WCC 2 Weeks to 1 Month
• Pediatric WCC 2 Months
• Pediatric WCC 4 Months
• Pediatric WCC 6 Months
• Pediatric WCC 9 Months
• Pediatric WCC 12 Months
• Pediatric WCC 15 Months
• Pediatric WCC 18 Months
• Pediatric WCC 24 Months
• Pediatric WCC 30 Months
• Pediatric WCC 3 Years
• Pediatric WCC 4 Years

Try It Out
Enter the Visit Information
This step is typically performed by support staff who will be rooming the patient. These two steps will set Epic
up to suggest the correct SmartSet.

SmartSets: Well Child Check

1. From the Rooming tab and open the Visit Info section.
2. Enter a Reason for Visit of Annual Visit, Well Child, or Weight Check.
a. Tip: Epic will use logic between the chief complaint and the patient’s age to suggest a SmartSet
in the next step. If a Chief Complaint is not entered, Epic will not suggest the well child check
SmartSet.

Open a SmartSet
SmartSets are primarily part of the provider workflow, but in some cases, support staff may help.
1. On the Plan tab, go to the SmartSets section.
a. Well Child Check SmartSets are suggested based on the patient's Reason for Visit.
2. Select the check box next to the SmartSet you want to use.
a. To add a SmartSet to your Favorites, right-click it and select Add to Favorites.
b. To preview a SmartSet before opening it, hover over it and click .
3. Click Open SmartSets.

Select items from a SmartSet


In a SmartSet, select the appropriate items for your patient. Some items might be preselected for you.
1. Click the name of a section to expand it.
2. Select the check box to the left of an item to document or order that item.

3. When you select an item, click the link, Order Details, to modify the details of the order. Clear the
check box to undo a selection.

4. To record more details about an item, hover over the item and click the link next to its name. For
example:
a. Click Details next to a diagnosis to mark a specific diagnosis as primary.

b. Click Add Now next to a note template to start editing the note. Do not stretch images and keep
within the margins.

c. If you're placing orders from a SmartSet, click an order's Summary Sentence to change the
order details.

5. As you are working through your Well Child Check SmartSet, pay close attention to the sections
labeled Progress Note, LOS & Diagnosis. These are three main components you need to sign and
close a patient office visit.
a. This is a SmartText for your progress note that follows Bright Future guidelines.

6. If you are placing medication orders - verify that the pharmacy listed at the bottom of the SmartSet is
correct. Scroll to the bottom of the SmartSet and click the link to update it.
7. To add an order that isn't included in the SmartSet, search for it at the bottom of the SmartSet, or select
it from your preference list. Orders added here are grouped with the rest of the SmartSet in reports.
8. When you're finished, click Sign. The information you recorded in the SmartSet now appears in the
appropriate places throughout the patient's chart.

Associate diagnoses with orders


1. When you sign orders, you'll be prompted to associate a diagnosis with any orders that require it. If the
diagnosis you need isn't listed, search for it in the Search for diagnosis field or, select it from the
Common, Previous, or Problems drop-down menus.
2. Click the cell where an order and diagnosis intersect to associate them.
3. Click Accept.
