Paraphilias: Recurrent, intensely arousing sexual fantasies, urges, or behaviors involving:

1) Nonhuman objects
2) Suffering or humiliation of oneself or a partner
3) Children or nonconsenting persons’
- that are not unusual considered sexually arousing by others

Diagnosis Criteria:
(PEFV) Pedophilia, voyeurism, exhibitionism, frotteurism - Diagnose if the individual has acted on
urges or if these cause marked distress or interpersonal difficulty.

Sexual sadism:- Diagnosis if acted upon urges with a nonconsenting person or if urges, fantasies, or
behaviors cause marked distress or interpersonal difficulty.

Other paraphilias:- Diagnosis if behavior, urges, or fantasies cause clinically significant distress or
impairment in social, occupational, or other important areas of functioning.

TYPES
Exhibitionism
- Exposure of genitals to strangers, sometimes with masturbation.
- Usually occurs before age 18; tends to be less severe after age 40.
-called flasher - feel a need to surprise, shock or impress their victims

- Fetishism
- Use of nonliving objects (fetishes) for sexual excitement or orgasm.
- Common objects include women's underwear, lingerie, shoes, etc.
- Begins in adolescence and tends to be chronic.
-partialism - sexually aroused by a body part such as feet, breasts, and buttocks

- Frotteurism
- Touching and rubbing against nonconsenting persons in crowded places.
- Common locations include public transportation and shopping malls.
- Most frequent between ages 15 and 25; frequency declines afterward.

- Pedophilia
- Sexual activity with prepubescent children (generally 13 years or younger) by someone at least 16
years old and 5 years older than the child.
- Involves various sexual acts, including fondling and penetration, undressing, exposing himself or
herself; masturbating
-Contact may involve the individual’s own children, stepchildren or relatives, or strangers.
- Many individuals do not experience distress about their fantasies or urges.

- Sexual Masochism
- Recurrent fantasies or urges involving humiliation, beating, binding, or suffering to achieve climax
- Can be acted upon alone or with a partner.

- Sexual Sadism
- Recurrent fantasies or urges involving acts that cause psychological or physical suffering to the
victim to achieve excitement
- Can include domination, restraint, and various forms of violence, humiliation and terror.
- Victims may be consenting or nonconsenting.

Transvestic Fetishism/ transvertism


- Recurrent fantasies or urges involving cross-dressing by heterosexual males.
-heterosexual males dressing female
-does not involve real partner
-individual fantasy that he is the female partner
-men wear female cloths
-cross dressing
-becomes problem when it necessary to become sexually aroused

Voyeurism (peeping tom)


- Recurrent fantasies or urges involving observing unsuspecting persons who are naked or engaging in
sexual activity.
- does not seek for physical contact
- Usually begins before age 15, is chronic, and may involve masturbation during the act.

Autogynephilia (wala sa book)


-men’s propensity to be sexual aroused by the thought of himself as female
- form of male to female (MtF) transexualism
-cross dressing and cross gender expression
- not involve women cloths

Asphyxiophilia/ hypophilia
-self strangulation form masturbatory purposes lead to unconcious or death

CAUSES
-childhood trauma
-frequent and repeated associated with a pleasurable sexual act

treatment
-CBT aversive conditioning - using negative stimuli to reduce or eliminate a behavior
empathy training - using the perspective of the victim
-pharmacologic intervention -such as
1) antidepressant
2) long acting gonadotropin realing hormones
3) antiandrogen - decrease testosterone
4) phenothiazines
5) mood stabilizer
