These forms may be used to add one new owner (accountholder) to your Citibank Checking, Savings and CD
accounts. Please use an additional Add an Owner kit for each owner.
What You Need to Know
Up to 5 total owners (and beneficiaries) are permitted on any 1 designated account and all owners:
• Must be 18 years of age or older.
• Must have a Social Security Number or Individual Taxpayer Identification Number (ITIN).
• Must have a current acceptable photo ID.
• Must be a U.S. Citizen or Permanent Resident Alien. Please visit a Citibank branch or contact CitiPhone Customer
Service at the number below for additional assistance.
• Must have a U.S. residential address. *Mail Drop or Post Office (P.O.) boxes are not acceptable unless a Army Post
Office (APO) or Fleet Post Office (FBO) box.
You can not use the Add an Owner Kit for the following accounts:
• Fiduciary accounts such as Estate Account, Formal Trust Account, or other Special Title Accounts include where
one person (fiduciary) manages funds for another person (beneficial owner), such as: Guardianships,
Conservatorships, Custodianships, Representative Payees, persons acting under a power of attorney or other
agency, and COMMA accounts.
• A linked Checking Plus account. If your account has a linked Checking Plus, please visit a Citibank branch or contact
CitiPhone Customer Service at the number below for assistance.
After the new owner is added to your designated accounts, the account title will change and the added owners will
have an ownership interest in the account and will be sent individual Citibank ATM/Debit cards.
Instructions
1. Complete all sections of the enclosed Account Title Change Form.
• One form should be completed for each new owner added to the account.
• All current accountholders must sign and date the form in the presence of a notary. For international notaries,
an Apostille is required.
2. Complete the enclosed Signature Card. *Top and bottom sections must be completed.
3. Complete the enclosed IRS Form W-9 (if a U.S. person).
• One form should be completed for each new owner added to the account. If you have questions about your tax
situation or how to complete IRS Form W-9, you may contact your tax advisor.
4. Attach a current photo ID for each new owner. (see Account Title Change Form for acceptable IDs).
5. Return the completed forms and copy of photo ID by mail to:

• These forms must be received within 90 days of the dated signature. If the forms are received after 90 days of
dated signature, Citibank will request that a new form be completed.

Important Information for Accountholders: To help the United States Government fight terrorism and money
laundering, Federal law requires us to obtain, verify and record information that identifies all accountholders authorized
to transact on the account.
What this means for you: When you are added as an accountholder, we will ask for your name, physical or residential
street address, date of birth, and an identification number, such as a social security number, that Federal law requires us
to obtain. We may also ask to see your driver's license or other identifying documents that will allow us to identify you.
