Michelle Sloan, 45, from Aston, was parked in front of theHalfords Autocentre reception to unload some parts. She was reaching into the back of the vehicle when a transit van reversed into her door, closing it and trapping herlegs. The door crushed both her knees causing a torn tendon inher left knee and a severed tendon in the right one. A yearafter the incident she is still struggling to walk and has been unable to return to work.

 An Auckland supermarket operator has been ordered topay $50,000 after a staff member's thumb got chopped ofin a meat slicer.  A deli assistant manager at Countdown Birkenhead wasdeboning ham on an automatic slicer in February whenher thumb hit the blade. The thumb later had to be amputated because of injuriessustained.

Butlers has been fined £235,000 after a Bristol man died while cleaning a pub. Richard Pratley, 65, who was employed as a cleaner at theSnuff Mill Harvester, died after falling 1.8 metres whilecleaning at a high level in an accident on 19 January 2009. He suffered a fractured skull and died from his injuries on23 January 2009.  A prosecution was brought by Bristol City Council's healthand safety and legal teams after an investigation led by one of the council's health and safety inspectors.
