Buckeye Community Hope Foundation eSchool Application Form

The final Application Process, Form and Rubric documents were posted on ODEs website and emailed to all sponsors and community schools on May 31, 2013. The final Application Process, Form and Rubric documents are required to be used and submitted to ODE by June 14 at 11:59 P.M. The eSchool application process is designed to determine an applicants eligibility and capacity to develop and operate a high quality eSchool. An eSchool, also referred to as an online school and defined as an internet or computer-based community school pursuant to Section 3314.02 of the Revised Code, is a community school in which the enrolled students work primarily from their residences on assignments in non-classroom-based learning opportunities provided via an internet- or other computer-based instructional method that does not rely on regular classroom instruction or via comprehensive instructional methods that include internet-based, other computer-based, and noncomputer-based learning opportunities. Online community school, eSchool and internet- or computerbased community school have the same meaning. For purposes of this application, an out-of-state school may be considered an eSchool if it is an online public charter school. Pursuant to Division 3314.02(A)(8) of the Ohio Revised Code, operator means either of the following: (1) an individual or organization that manages the daily operations of a community school pursuant to a contract between the operator and the schools governing authority; or (2) a nonprofit organization that provides programmatic oversight and support to a community school under a contract with the schools governing authority and that retains the right to terminate its affiliation with the school if the school fails to meet the organizations quality standards. For purposes of this application, an out-of-state school may be considered a community school if it is a public charter school. eSchool Application information Applicants will be evaluated based on their assessed ability to meet the statutory measures as defined by Ohio Administrative Code Section 3301-102-09 (A)(7). The five measures below will be used to assess the experience and quality of applicants to develop new internet or computer-based community schools per Section 3314.013 of the Ohio Revised Code: I. Buckeye Community Hope Foundation is desirous of sponsoring Buckeye Urban Education Solutions (BUES) dba Insight School of Ohio because of their focus on student outcomes and student support. Buckeye Community Hope Foundation recognizes that employers are looking for candidates with a blending of specific skills and content knowledge that includes a heavy emphasis on the use of technology. Insight School of Ohio will provide students with skills for living in the real world. Students will master multi-dimensional abilities suited to the learners pace and method of learning. The school allows the teachers to be creative and collaborative with the students providing on-line tools literally at their fingertips. All of this is supported by a proven operator, K, who has a successful track record both in Ohio and nationally. The mission of Buckeye Urban Education Solutions is to support a school that prepares students for being successful in college, a career, and to thrive in a diverse community through an engaging blended learning experience that supports individual students needs in order to help them attain their maximum potential. In support of BUESs mission, Insight School of Ohios mission is to keep students in the public school system and help them to reach their individual potential. Insight School of Ohio will provide an excellent education alternative and student supports for students throughout Ohio who, for a variety of reasons, do not attend school, or who may benefit from a non-traditional school setting. The educational program and philosophy of Insight School of Ohio is structured to support these students towards earning their high school diploma. All students are provided instructional strategies rooted in scientifically proven methods supported by data in order to improve student achievement effectively. Live online instructional sessions are provided daily. Online sessions provide direct instruction on course specific objectives and assignments, small group and one-on-one targeted intervention support based on students identified areas of need, and open office hours and tutoring support in a drop in format for math, English, history, and science. Insight School of Ohio will be a new start-up K-12 online community school open to any student residing in Ohio. The primary location for Insight School of Ohio will be in the Columbus City Schools District. For the first year of operation, the school will serve grades 6-12.

Electronic Classroom of Tomorrow (ECOT) - Statewide K-12 Public On-Line Community School July 2002 August 2004 Served temporarily in the role of Controller for the first (6) months while the Board looked for a permanent candidate. Worked as Program Administrator primarily responsible for NCLB compliance and Federal Programs, specifically: Title I-School Improvement, Title IIA-Improving Teacher Quality, Title II-D Technology, Title IV-A Safe and Drug Free Schools, and Title V Innovative Programs. Ensured all Federal Programs listed above were utilized, documented, and reported in accordance with Federal and State laws in a manner that supported student outcomes. Planned, coordinated, and executed professional development regionally around the State for over 200 teachers and staff. Planned, coordinated, and executed face-to-face parent-teacher conferences in conjunction with regional teacher professional development meetings. Planned, coordinated, and executed nurse led vision and hearing screenings, in conjunction with, parent-teacher conferences regionally. Served as the LPDC Chairperson for ECOT. Planned all LPDC meetings, kept record of minutes, and monitored over 200 teacher credentials and renewals for NCLB compliance. Purchased, set-up, and administered the State of Ohio WebCheck background system on-site at the school. Conducted all fingerprint scans for new employees, existing staff, and teacher renewals. Monitored compliance with Federal and State rule and law. Conducted monthly site visits. Reviewed student, special education, and staff files for compliance. Provided technical assistance to School leadership and the Governing Authority. Attended all Board meetings. Attended pre- and post- Auditor of State meetings of ECOT. Represented Lucas County ESC at ECOT graduation. Reviewed financials to ensure fiscal compliance and viability. Fielded parent complaints and worked with ECOT to resolve timely.

Credentials related to Online and Blended Learning David Massa is a Ohio licensed School Treasurer contracted to conduct independent financial reviews and monthly Board reports for Buckeye Community Hope Foundation sponsored schools. Dave utilizes a comprehensive system to provide BCHF with a clear picture of the enrollment and fiscal performance of each school we sponsor and helps us to provide our sponsored school Boards with feedback on a monthly basis. Below is a summary of his experience directly related to our capacity as the Sponsor of Insight School of Ohio. Works as the School Treasurer for Ohio Connections Academy, a K-12 on-line school in Ohio; operated by Connections Education, a successful multi-state operator. Previously worked at as the School Treasurer of the Ohio Distance and Electronic Learning Academy (ODELA), a K-12 virtual school in Ohio.

Below is a summary of his experience as an e-school Treasurer that directly relates to our capacity as the Sponsor of Insight School of Ohio. Assists in the development, implementation and maintenance of the fiscal policies and procedures for the School and the Governing Authority in accordance with professional accounting standards; Maintains financial stability in internal fiscal controls and systems to assure compliance with established standards, policies and procedures; Provides technical assistance to the School and Governing Authority. Maintains all financial records in accordance with Generally Accepted Accounting Principles (GAAP); Assists in preparation, monitoring, and revision of Annual Budget for the School. Assists in the preparation, revision, and submission of the Schools Five Year Forecast in accordance with Section 5705.391 of the Ohio Revised Code; Communicates with the Ohio Department of Education and the Auditor of the State of Ohio, among other funding agencies. Responsible for financial reporting and cash draw down requests for all federal and state grants made to the School; Ensures compliance with purchasing procedures, perform physical inventory of assets and equipment annually, maintain current inventory list, enter items are tagged as purchased or obtained; Maintains appropriate depreciation schedules for capitalized assets; Annually reviews service contracts, leases, maintenance agreements, group health and life insurance, vehicle insurance, commercial insurance and workers compensation insurance, requesting bids for necessary; Coordinates and act as the liaison between the Governing Authority, School, and Auditor of State of Ohio during the annual audit process. Buckeye Community Hope Foundation Education Division School Improvement Specialist Dr. Carol S. Young

Credentials related to Online and Blended Learning Dr. Young oversees all School Improvement activities at Buckeye Community Hope Foundation. Dr. Young collects, analyzes, and reports to the Education Division leadership-the status of the academic performance for all schools in our portfolio. She provides professional development to both school Administrators and Teachers related to student performance and student outcomes. Dr. Young works in conjunction with each regional sponsor representative to monitor academic performance throughout the school year, school improvement planning and implementation, and the schools performance in meeting the sponsorship contract performance measures for renewal. Below is a summary of her experience directly related to our capacity as the Sponsor of Insight School of Ohio. Training Completed 9 hours of graduate coursework in facilitating online learning, designing online courseware, and online assessment (Ashland University; Mount Vernon Nazarene University).

K-12 Public Schools (Traditional and Community) Partnered with the vendor, Virtual Community Schools, to offer online and blended courses for credit recovery, homebound learning, and accelerated learning in a traditional public school environment. Assisted with training and preparation of 7-12 teachers to facilitate synchronous and asynchronous online learning for credit recovery, home bound learning, and accelerated learning. Modified courseware to meet adaptive technology needs of students with disabilities. Monitored and evaluated student achievement results of the partnership. Performed regular compliance monitoring of 3 drop out recovery schools utilizing online and blended course delivery. Evaluated student achievement data drawn from online courseware.

Prior to joining our sponsorship team, Mr. Devers worked at the Ohio Department of Education in the Office of Exceptional Children.

K-12 Public Schools-Community Worked as the Principal and Technology Coordinator for (5) years at a K-7 community school in Ohio. Served as the Network Administrator. Managed the supplemental e-learning component Compass Learning Odyssey. Configured and monitored internet content filters for students. Developed the schools technology plan. Evaluated and selected all vendors related to technology equipment, curriculum, and installation. Set-up all student and staff profiles in the e-learning system. Monitored student progress and reports. Provided training and technical assistance to students and staff in the e-learning system.

In September 2001, after 18 months of research and development of our curriculum, K12 Inc., introduced our kindergarten through 2nd grade offering. We launched our initial online learning system in virtual public schools in Pennsylvania and Colorado, serving approximately 900 students in the two states combined. We added new grades over the first seven years and continue to manage schools in more states every year. We have also launched hybrid programs that combine face-to-face time in the classroom with online instruction and opened an online private school to reach students worldwide. For the 2010-11 school year, K12 Inc., managed schools in 27 states and through our Institutional Business served schools in all 50 states. In 2011-2012, K12 Inc. served new statewide public virtual schools in Louisiana, Tennessee, and Indiana and new blended learning (combining online and face to face instruction) schools and programs in Arizona, California, Colorado, Kansas, Michigan, Oklahoma, Pennsylvania, Texas, Utah, and Wyoming. III. The academic performance of the sponsors schools in all years under its sponsorship (25 possible points) For this section, ODE will use the sponsors Performance Index Score, per Section 3314.016 of the Revised Code as found on the Office of Community Schools webpage. ODE will use community school academic data drawn from the Local Report Card as presented in the Annual Report on Community Schools, Table 1, on the Office of Community Schools webpage IV. The academic performance record of the operators affiliated schools records in all years under its affiliation (25 possible points) List all Ohio community schools with which the operator has been affiliated and the calendar years of that affiliation. For this section, community schools academic data will be drawn from the Local Report Card, as presented in the Annual Report on Community Schools, Table 1, on the Office of Community Schools webpage.

For all affiliated out-of-state public charter schools, provide academic data and applicable state performance ratings assigned to the school for the most recent academic year. In K12s early years, academic performance in K12-managed public schools was largely within norms as measured by state assessment tests. Recently, however, the number of families choosing to enroll their children in K12-managed public schools has increased rapidly, and many of those children are academically at-riskthat is, they are one or more years behind grade level, according to state test scores or other data. In the larger K12-managed public schools that we have analyzed, academic performance for incoming students is well behind grade level. We estimate that as many as 50% to 70% of new students did not achieve proficiency in math on state exams taken in the year before enrolling in a K12-managed public school, and up to 40% did not achieve proficiency in reading. K12-managed public schools are also being chosen by large numbers of high school students who are not on track for on-time graduationin one sampling of these schools, approximately 40% to 60% of incoming 10th, 11th, and 12th graders were credit deficient upon enrollment. The Insight program is designed to address the needs of the increasing number of high school students coming in who are severely credit-deficient or are experiencing life circumstances placing them at risk either of not graduating with their cohort or of dropping out of school. The Ohio Virtual Academy opened its doors in fall of 2002, and K12 Inc. has been the education management organization from the start. Academically, the Ohio Virtual Academy has performed at the Continuous Improvement or Effective levels for each year since the performance ratings have been in place. Please see the Ohio Virtual Academy report cards attached as OHVA Report Cards. Since its founding more than a decade ago, K12 has been inspired by the belief that technology can allow any child to obtain a high-quality education regardless of location or economic circumstances. As K12 has grown, we have embraced the potential of technology to enable the transformation to truly individualized learning. We believe the industry is reaching an exciting inflection point and day- byday, K12 is working to rise to the challenge. The purpose of this report is to provide a clear-eyed view of our progress, areas where improvement is still needed, and efforts we are taking to close the gap. K12 and its charter school and school district partners are leading the transformation to individualized learning, with curriculum and services used in 85 countries on six continents, including more than 50 physical sites. Our courses are offered in full-time online schools, blended schools, traditional classrooms, pre-kindergarten classrooms, and homeschool environments. We now serve more than 110,000 full-time students in public schools managed by K12 in 33 states and the District of Columbia. These K12-managed public schools are open to every child based on choice, not geographic location or economic means. For most students, K12-managed public schools are the only public school alternative available. The Ohio Virtual Academy opened its doors in fall of 2002, and K12 Inc. has been the education management organization from the start. Over the past 11 years, K12 Inc. has grown with Ohio Virtual Academy in enrollment numbers, grade levels served, and services provided to students. In its first year, Ohio Virtual Academy served students in grades K-5, expanding to grade 6 for the 2003-04 school year, adding grades 7 and 8 during the 2004-05 school year, and adding high school during the 2005-06 school year. Today, Ohio Virtual Academy serves over 14,000 students in grades K-12.

Each application will be rated using the criteria included in the eSchool Application Rubric and receive a composite score by adding the scores from each of the 5 sections in the rubric. In addition to the composite score, information pertaining to sponsor and operator experience in terms of statutory compliance will be reviewed. Each applicants awarded point value for each of the five measures, the composite score, and information about the sponsor and operator experience in terms of statutory compliance will be considered by the Superintendent of Public Instruction in making decisions regarding the approval or disapproval of submitted and reviewed applications. Decisions regarding approval and disapproval are based on an applicants scores on measures of experience and quality per Section 3314.013 of the Ohio Revised Code.
