Dear Chief Clerk Israel Hen:
The Regulations of Legal Competency and Conservatorship (Court Procedures and
Execution), 1970 (combined version), say:
Decree Appointing a Conservator, amendment 1988
9 (a) The decree appointing a conservator shall be drafted pursuant to Form No 1 in
the Appendix, and shall be marked by consecutive serial number of the court that
issued the decree...
The copy of the Decree Appointing a Conservator, referenced above, [1] which was provided
to me in the Office of the Clerk, is not marked by a consecutive serial number, as prescribed
in the Regulations, copied above.
Therefore, I request that you immediately provide the consecutive serial number of the
Decree, referenced above.
Separately, I again request that you provide a copy of your due appointment record as Chief
Clerk of the Haifa Magistrate Court with your response.
