Dear Sirs, Re: Waiver, Release and Indemnity Agreement for the participation of Lions Club of Petaling Jaya and Leo Club of Petaling Jaya Integrity (O) For and in consideration of the Lions Club of Petaling Jaya and/or Leo Club of Petaling Jaya Integrity (O) permitting me: Name: NRIC No.: (hereinafter referred to as the Participant) to enrol in and participate in the Leo Orientation Camp 2011 to be held at Taman Wetland Putrajaya, commencing from 26 th August 2011 to 28th August 2011, jointly organized by the Lion's Club of Petaling Jaya & Leo Club of Petaling Jaya Integrity (O), I, hereby voluntarily release, discharge, waive and relinquish any and all actions or causes of action for personal injury or wrongful death occurring to the Participant arising as a result of enrolling, participating in the above said Leo Orientation Camp 2011 or any activity incidental thereto wherever or however the same may occur and for whatever period the said activities may continue and the undersigned does hereby releases, discharges, waives and relinquishes any action or causes of action aforesaid, which may hereafter arise to the Participant, and agrees that under no circumstances will he/she prosecute or present any claims for personal injury or wrongful death sustained or occurred to the Participant against its Officers, Agents, Servants or Employees, its Members, Agents or Servants for any of the said causes of the said person or otherwise. Presentation Break Camp

: All Participating Leo clubs are require to prepare some items for presentation on fellowship night on the 2nd night (approx 3 to 4 item) : Participants are not allowed to leave the camp premises of halfway without parents/guardian verbal/ letter of consent unless he/she in medically unfit. : Transportation will be included. A bus will depart from A&W Taman Jaya at 2.00pm.Participants will be required to be in A&W by 1.30pm sharp on the 26th of August 2011 (Friday). Separate transportation will be provided for Muslim Leos who are having Friday prayers. : All Clubs are to bring along their own club banner for photograph taking and club identity

Key officers of all participating Leo clubs are encouraged to attend this camp.

Only a maximum number of 10 participants from each club are allowed, but however, if the number of participants exceeds 10, the remaining participants will be in a waiting list. There will be Lion Officers and Omega Leos in attendance to oversee the Leos throughout the duration of the camp. Valuables are brought at own risk, organizing committees are not held responsible.
