PART II – PERFORMANCE RATING

 In this part employee is rated according to


his/her performance and assigned
responsibility completion.

– Achieved excellence
– Exceptional Achievement
PART III – GENERAL BEHAVIOR
The employee is rated on the behavioral traits
shown in the routine work, these traits can be

 Interpersonal Skills
 Attendance/ punctuality
 Orderliness
PART IV – ACTION PLANS

Action plans are stated depending upon


different levels

– For achieving expectations


consider timetables, workshops, setting
interim goals, etc

– For achieving excellence

– For career development


PART V – For Employee

 Employee has given the opportunity to state


his/her comments on the ranking/rating of
supervisor or rater.

 More over employee is allowed to submit the


training application, as if employee feels to get
trained in specific field.
This would encouraging by the University.
