Dear Sir,

Policy # 4034i/FPP/04676377/00/000 Period:6/5/2010 to 5/5/2011 Claim # 220100149927

This is regarding my late father s claim for reimbursement on the above policy. Th e claim was filed on April 2011. The first set of original documents were sent t o you on 2nd May 2011. There was a set of mails from the Lombard authorities asking for more supporting documentation/proof as regards age etc. which was duly complied.

However the claim was casually rejected due to incorrect age of my father provid ed in the policy details.

My father's Date of Birth was 16 September 1950. (It was wrongfully given as 16 September 1953 , due to miscommunication through your tele-call services) Also, my father had a 2 year policy in ICICI Lombard previously (4034/FPN/002729 73/00/000) which had his correct Date of Birth.

How it is possible for the same insurance company that during canvassing of busi ness you take correct date of birth for the first two years and subsequent rene wals, untrained tele -callers note down some other date of birth. They have nev er bothered to get the existing data which was very much available with your off ice when the policy was first taken, this casual attitude resulting in causing u ntold agony at this hour of need.

The Thane office of ICICI Lombard was constantly in touch with my late father a nd had confirmed that the modification for age would be done on the next renewal so that the cumulative bonus could be given. This has been confirmed by your Th ane office. (My father passed away on 4th April 2011, before the renewal date 5th May 2011) Due to the incompetence of your officials, this was never changed. And you have used this fact to reject our claim. This is not a satisfactory explanation.

It is surprising how casually you treat the veracity of the claim

Please let us know why our case wasn t considered in spite of the above findings?
