    Waiver of Liability and Permission to Photograph: I, the undersigned, understand that
    Kaw Valley Quilters Guild and the Quilt Show will take every precaution for the
    security and safety of this item during the set-up time and quilt show, but cannot be
    responsible for damage or loss to any item loaned for the exhibit. I also grant permission
    for the Quilt Guild to digitally photograph my work for inclusion on the website and/or
    on a CD-Rom, available for sale.

    Quilts must have a sleeve large enough for a 1 rod and a label identifying the
    submitter.
    Quilts must be delivered to the NEW SHOW LOCATION at the CROWN
    TOYOTA DEALERSHIP, 3401 Iowa on Friday, April 5 between noon and 5:30
    p.m. If you cannot do this, contact Georgann Eglinski, 785-842-8625,
    geglinski@ku.edu to make other arrangements.
    Quilts must be picked up at the NEW SHOW LOCATION at the CROWN
    TOYOTA DEALSERSHIP, 3430 Iowa on Sunday, April 7 at 5 p.m.
