#1 Nursing Diagnosis: Acute pain related to labor as manifested by grimacing face, guarding behavior, irritable, sweeting and verbalization

Acute pain can be described as lasting from seconds to 6 months. The patient is pregnant and she is in her labor stage in which he feels uterine contraction, descend of her baby on her vagina thus she feels discomfort and acute pain during contraction. Nursing Goal: After 45-50 minutes of giving appropriate nursing interventions, the patients will have decrease manifestation of grimacing face, guarding behavior, irritable, sweeting and verbalization of haan unay nakasakit ti tiyan ko no agpatangken ni baby. Nursing Interventions 1. Position the client Rationale To provide nonpharmacologic pain management 2. Provide quiet environment 3. Provide calm activities 4. Teach to do relaxation exercises 5. do deep breathing exercises To decrease feeling of irritability To decrease the pain felt by the patient To decrease pain to decrease pain

Evaluation: After 50 minutes of giving appropriate nursing interventions, the patients was able to have decrease manifestation of grimacing face, guarding behavior, irritable, sweeting and verbalization of haan unay nakasakit ti tiyan ko no agpatangken ni baby.
