History a.

Present Illness On July 2012, seven months prior to admission, the patient experiences sudden epigastric pain aggravated by eating large amount of fatty and salty foods and relieved by drinking a lot of water when the pain begins. This symptom then made her decide to go for consultation at Baguio General Hospital and Medical Center (BGHMC). b. Past Illness On 2006, she was pregnant for her 4th child but decided to have abortion with a manghihilot. She said that she had done this because she just delivered her 3rd child who is 3 months old that time. After having abortion, she experienced severe vaginal bleeding and was then sent to Urdaneta Sacred Heart Hospital. She woke up and the doctor said she had undergone total abdominal hysterectomy bilateral salpingo-oophorectomy due to uterine perforation and ruptured ectopic pregnancy. Medications was given but was unable to recall the said drugs. On 2008, she went to BGHMC for consultation due to presence of small lump on her right breast. She had breast biopsy but was diagnosed to be benign. Still, medications was given but was unable to recall the said drugs. On January 2011, she went back to BGHMC for

consultation still due to the lump on her right breast since it proliferated with a pus-like discharge coming out from it. She was then diagnosed to have right breast cancer stage I-B. She had undergone 6 cycles of

chemotherapy and has taken propan with iron. She claimed to have no history of allergies. c. Family History She has a family history of hypertension on her

mother side and asthma on both parents side. She has also a history of diabetes mellitus on her father side. d. Social History She is a housewife since she was operated for

mastectomy. She is married to a farmer at the same time a driver and blessed with 3 children; also, she is

affiliated to Roman Catholic. She started drinking liquor and smoking at the age of 15 with 5 sticks and progressed to one pack of cigarette per day. She said that her husband is also drinking liquor but not smoking. Last January 2011, she stopped smoking cigarette as well as stopped drinking alcohol due to present condition.
