AFFIDAVIT OF UNDERTAKING I, HENRY A. AGAN, PRESIDENT of HOLY CHILD COLLEGE OF INFORMATION TECHNOLOGY, INC., with office address at ALLAH VALLEY DRIVE, SURALLAH, SOUTH COTABATO, after having been sworn into an oath in accordance with the law do hereby depose and say that in accordance with the Implementing Guidelines on the DSWD-TESDA Cash for Training Project embodied in TESDA Circular No. 33 Series of 2012, issued on November 23, 2012, hereby commit the following: 1. Promote and advocate the Cash for Training Project in enhancing the competitiveness and productivity of the workforce through skill training programs. 2. Conduct, in coordination with the Provincial/District Director, the Training Induction Program before the conduct of any training. 3. Submit to the Provincial/District Director the Enrolment Report, Terminal Report, Assessment Report and Employment Report using the prescribed MIS-03-02 within (5) calendar days after the start and end of training. 4. Not to pass-on additional fees to the scholars. 5. Motivate and nurture the scholars to maintain one hundred per cent (100%) completion of the training program. 6. Allow at least eighty per cent (80%) attendance and participation to qualify for completion of the training program. 7. Agree that payment of the training fees shall be based on the number of scholars who have completed the training program. 8. Ensure that all scholars undergo the appropriate assessment, whenever applicable, upon completion of the training program in coordination with the Provincial/Director. 9. Institutionalize appropriate activity to mark the completion of every training program and report the same to the Provincial/Director.
