Case 1 A doctors chamber requires a database for effective patient and vaccine management.

Each doctor treats more than one patient. One patient can be treated by many doctors. When treated, the advice of the doctor is written in a prescription along with the date and age of the patient (calculated from his date of birth). One patient may take many vaccines. One vaccine may be taken by many patients. Besides doctors there are also general employees in the chamber. A patient is given a specific vaccination by an employee. When a patient is given a vaccination by an employee the date and time is also stored. Doctor: Doctor ID, Name, Specialty (can be more than one) Patient: Patient ID, Name, Date of Birth (composed of day, month, year) Vaccine: Vaccine ID, Name, Batch no Employee: Employee ID, Name Case 2 A database is required to keep track of employees, their skills, spouse, projects assigned, and departments. Every employee has a unique number. If an employee is married to another employee, the date of marriage and who is married to whom is also stored. However no record of the spouse needs to be stored if the spouse is not also an employee. There are 10 departments. An employee reports to only one department. One employee works in only one department. Many employees work on a project. An employee can work on many projects. Employee: Employee ID, Name, Hiring Date, Skill (can be more than one) Project: Project ID, Name, Date Started Department: Department ID, Name, Location Case 3 There are members in bands. One member is member of exactly one band. One band must have at least one member. Bands release albums. An album is released by a band. A band may release as many albums they wish. A song may be included in many albums. One album must have at least one song. A member of the band works as a band leader of the band. Each band can have only one band leader. The date when a member is appointed as the leader of a band is also needs to be stored. A song is identified by a song ID and can have a title and a category. Members identifier is member ID and they have name, date of birth and age (is derived from date of birth). An album is identified by album ID and can have album title and year released (can be multiple values if the album is re-released). A band is identified by Band ID. A band also has band name and year established attributes.
