Terms of Reference
Purpose: The Reference Group provides advice and guidance to ACME Comorbidity in the implementation of its Improved Services Initiative project that enhances and supports its capacity, specifically as it relates to developing sustainable partnerships with the broader health and community support networks. The Reference Group provides a forum for informed discussion on the implementation of the ACMEs Improved Services Initiative with particular emphasis on ensuring the integrated and timely roll out of the Project by insert date in Victoria. It provides the co-ordination point for the implementation of Improved Services Initiative in Victoria. The Reference Group provides guidance and the authorising environment to progress the implementation across Victoria. Membership: The Reference Group comprises representatives from Community Service Organisations, Service Providers, and XXX in the Victoria. (Refer to attached list) ISRG members bring experience and knowledge of their respective areas; access to major networks, capacity to influence and disseminate information and ability to comment on social and health policy, service development and implementation. Meetings: The Reference Group members will determine dates. The Reference Group will meet at least bimonthly. The ACME Reference Group will exist for the life of the project. Authority The Reference Group reports to ACME as the Project Manager. ACME authorises final versions of any reports and submissions that arise from this project. Public statements in relation to this project or the workings of the Reference Group will be authorised by ACME.

Roles and Responsibilities: Provide strategic direction and advice on issues and key themes emerging from implementation of Improved Services Initiative within Victoria. Monitor the implementation of the Victorian work plan and the development of strategies and tasks identified in the work plan. Debate, clarify, comment and make recommendations on draft policies and guidelines to reflect local needs. Identify overall implementation issues, risks and gaps likely to impact on the Victorian implementation and work towards solution-based outcomes. Provide feedback and strategic advice on progress and milestones, issues resolution and policy directions to progress the implementation. Oversee and monitor the work undertaken by the three Sub-Committees. Take carriage of the work required for the XXXX element of the project. Advise on future monitoring mechanisms for the implementation of the project. To act as a problem-solving forum to address issues that arise at either a policy or operational level To identify strategies to mitigate emerging issues associated with implementation of the project
